Please note:  DML requires an initial Deposit of $250 be made before any processing of the Direct Mail Campaign begins.  

If you wish to pay for Mail Services by Credit Card we require this form to be completed for each Transaction.

Please note: You may use this form to pay the Invoice for your Campaign but you must list the Invoice # and amount on the Form. Note there is a 4% Credit Card fee for Postage or Freight as DML doesn't markup . You pay what we pay.

1. The completed signed Credit Card Authorization form along           with a copy of your Photo ID and front of the Credit Card. 

2.  Your mail List in Excel (xls) Format.

3.  Your Letter for Envelope Mailing or copy for Postcard in word         document ( doc.x) Format.
